Program Description:
This HMO-based program is 4-6 weeks in duration and based on the "Stages of Change" model of motivational counseling.  Materials are produced/created by Sentara and services are provided by Health Educators and Registered Nurses (all Master's prepared facilitators).  Services are provided for physician practices, home health, high-risk pregnancy, hospital patients, etc.

Program Services:
Face-to-face counseling, group counseling, telephone counseling, inpatient services, web-based support and relapse prevention (for recent quitters).  Program site is wheelchair accessible and accessible by public transportation.  Program fees are reimbursable by insurance through the HMO offering the program (Optima Healthplan and Sentara Family Care) and nonmembers are also eligible for a fee.  Services are available for cigarette, cigar and pipe smokers and smokeless tobacco users, with special focus on pregnant women and low-income populations.
